How to refer

  1. Call usCall 801-278-3636. For a patient in pain, we make every effort to see them the same day.
  2. Send the detailsAsk us for our referral form, then send it back by fax with any x-rays you would like us to see. We can also take the referral over the phone.
  3. We report backAfter treatment you receive a full report, and your patient returns to you for the crown or permanent restoration.
